This collection of resources is not exhaustive. Among the dozens of PCMH efforts and resources available, these are the resources most often recommended by the practice facilitators who helped develop this curriculum. The list is not meant to be an endorsement of any specific tool or program.

The Agency for Healthcare Research and Quality offers a helpful how-to guide for organizations interested in starting a practice facilitation program aimed at improving primary care. It includes an extensive collection of tools and links to help you develop and run a practice facilitation program, including job descriptions.

Honing your practice facilitation skills

Coach Medical Home specifically focuses on guiding practices to implement the Safety Net Medical Home Initiative Framework for Practice Transformation. Here are some resources for developing general coaching skills:

Ongoing Research

In light of the large number of medical home pilots and evaluations, the Commonwealth Fund established the Patient-Centered Medical Home Evaluators' Collaborative in 2009 to align evaluation methods to the greatest extent possible, share best practices, and exchange information to improve evaluation designs. The collaborative is comprised of researchers who are engaged in assessing patient-centered medical home (PCMH) demonstrations and are unaffiliated with groups promoting the PCMH.
