
Becoming a patient-centered medical home (PCMH) is a major undertaking for any practice. The Coach Medical Home curriculum was designed to equip practice facilitators with public domain tools and knowledge to effectively support practices on the journey toward medical home transformation and recognition. The curriculum is based on the Safety Net Medical Home Initiative Framework for Practice Transformation, which consists of eight change concepts to guide primary care transformation in a variety of settings.
A special supplement to the journal Medical Care presents the progress and lessons learned from the SNMHI. One of these articles focuses on the development of the Coach Medical Home curriculum. Access the article here.
